A slight correction, the automatic decrimentation occurs if you use Always run exploits" not Scan if ping fails.

Be careful using this option though, since all IP's scanned whether truly reachable or truly unreachable will be decremented from your key.

Use the Tools-Options to check the SCAN IF PING FAILS option. This will allow to scan the system even if it is not reachable through ICMP

It sounds like you might have ICMP blocked from the public side. If you
are running some firewall software that is able to block ICMP (iptables,
ipchains, zone alarm), this could well be the case. You are blocking one
interface, and not the other. Allow ICMP, and you should be able to scan.
